JOB TITLE: Social Worker - Supporting Adults with an Intellectual
Disability.
JOB LOCATION(S): SOS Kilkenny Clg. (based in Kilkenny city).
REPORTING TO: Support Services Manager.
CONTRACT TYPE: One Year Fixed Term
HOURS: **Apply on the website**.5 Hours per week.
SALARY: Linked to Professionally Qualified Social Worker HSE Grade.
ROLE: SOS Kilkenny clg are seeking an energetic, flexible, innovative
and enthusiastic Social Worker to play an integral and active role in
a committed team to make a significant contribution in the area of
social work.
RESPONSIBILITIES INCLUDE:
* Ensuring that the rights of the people we support and their
families are respected.
* Assessment, planning, implementation and evaluation of social work
programmes according to professional standards and to keep abreast of
developments in the field.
* Manage interactions to ensure the highest professional standards
using an evidence-based, care planning approach.
* Conduct comprehensive assessments of persons with an intellectual
disability to determine the most appropriate social work methods of
practices that would best enhance the quality of life of the person
with an intellectual disability.
* Assist in the identification of perceived personal and social
needs to develop a greater awareness of the personal and social needs
of the person with an intellectual disability.
* Collaboration with person supported, family and other staff in
goal setting and support/intervention planning.
* Participate in the multi-disciplinary/healthcare team and work in
co-operation and collaboration with team members.
* Maintain comprehensive records of assessments, interventions
carried out, communications relating to the person supported.
* Carry a case load, including a greater complexity of
individual/family need.
* Inform the individual with an intellectual disability and their
family of their rights, entitlements, benefits and services.
* Act as an advocate for persons with an intellectual disability
where appropriate.
* Ensure that the appropriate relevant guidelines are adhered to in
the area of safeguarding vulnerable persons, in accordance with HSE &
governing bodies.
* Carry out the responsibilities of the Designated Officer and
Designated Liaison person as required.
* Ensure the highest standards of professional practice are
maintained in the Social Work Department in line with current
legislation and acting in accordance with organisational guidelines
and procedures.
* Provide relevant data and documentation on population profile
within designated practice areas.
* Maintain and promote good communication and relationships with all
other disciplines in the services and working with the agreed policies
of the organisation.
* Participate in case conferences, case reviews, team meetings and
other meetings as required.
* Participate in the admissions, transfer and discharge processes as
required.
* Committee Membership: Restrictive Practice Committee.
* Supporting business case funding applications.
* Carrying out such other duties as may be assigned from time to
time.
PROFESSIONAL QUALIFICATIONS / EXPERIENCE CRITERIA
_ESSENTIAL:_
* The person appointed will hold a professional qualification in
Social Work (NQSW/CQSW) or equivalent qualification and be eligible
for membership with the Irish Association of Social Workers.
* Applicants must be registered with or in the process of
registering with CORU.
* Have at least 2 years relevant post qualification experience in
Social Work.
* Energetic individual with experience of working in a diverse
multi-disciplinary environment.
* Demonstrate effective interpersonal, communications,
organisational and planning skills.
* Competent and confident IT Skills.
* Full Clean Driving Licence.
_DESIRABLE:_
* Experience in working with people with Intellectual Disabilities
would be an advantage.
